Please be reminded that in accordance with Veolia | Water Tech (“VWTS”) Terms and Conditions, all VWTS entities and affiliates worldwide require Certificates of Origin (“COO”) for all shipments. For your reference at the following https://www.watertechnologies.com/supplier-documents, is a sample Certificates of Origin with the requisite information in a format acceptable to VWTS. While VWTS suppliers/vendors may use alternate forms, at a minimum, the following information must include:
• 出口商/销售商的名称和地址

• 原产国
• 实际制造商的省、州、地区或地址
• 自由贸易优惠标准（如适用）
• 适当时，可能需要提供磨坊证书

The Certificate of Origin should be forwarded to Veolia | Water Tech along with the other required Shipping Documents, which include: Commercial Invoices, Packing Lists, Bills of Lading, Importer Security Filing (ISF) forms, and Certificates of Analysis.
VWTS将不负责任何国家的海关机构拒绝入境的商品或任何由此产生的费用，包括但不限于滞期费、关税、消费税、运费、经纪费或供应商发票，所有这些均应由供应商/供应商负责。
